Social Services | Clinical Resource Management
The professional nurses and social workers who staff the hospital’s Clinical Resource Management Department assess patients and work with hospital staff to coordinate their care from pre-admission through hospitalization to discharge. They serve as a link between hospital staff and departments and to outside agencies and facilities making referrals when appropriate and providing individualized attention to insure that each patient’s unique needs are met – including those for spiritual and pastoral care.

- Referrals for home care, rehabilitation or nursing home placement, or Pastoral care
